Once you have received your acceptance notification, please submit the items below. Items 1 to 5 can only be completed via a registration link provided to you by Aquinas Academy. Items 6 to 10 should be downloaded below, printed, completed and returned via email to [email protected] . **Items 11 to 13 should come from personal family records, etc.

A complete registration includes the following items:

1. Completed registration form (personalized link)
2. Tuition Agreement (on registration form)
3. New Student Agreement (on registration form)
4. Non-refundable registration fee of $350.00 (Paid with registration)
5. Non-refundable tuition deposit of $500 per student (Paid with registration)
6. Archdiocesan Permanent Record Card
7. Official Release Request Form (applicable to Grades 1-8)
8. Catechetical Family Contact Form (if Roman Catholic)
9. Health Requirement (physical) Form (must submit by the 1st day of school)
10. B6T Transportation Form (applicable to Kindergarten - Grade 8)
11. A copy of the student’s birth certificate (**already supplied  with Application)
12. A copy of the student’s Roman Catholic baptismal certificate (**already supplied with application, if applicable)
13. Immunization Records (**already supplied with Application. Any updates must submit by the 1st day of school)
